Emerging risk factors for atherosclerotic vascular disease: a critical review of the evidence.

چکیده
CONTEXT Atherosclerotic vascular disease is an enormous public health problem. A number of emerging risk factors for atherosclerosis have recently been proposed to help identify high-risk individuals. OBJECTIVE To review the epidemiological, basic science, and clinical trial evidence concerning 4 emerging risk factors: C-reactive protein, lipoprotein(a), fibrinogen, and homocysteine. DATA SOURCES Using the terms atherosclerosis, cardiovascular disease, risk factors, prevention, screening, C-reactive protein, lipoprotein(a), fibrinogen, and homocysteine, we searched the MEDLINE database from January 1990 to January 2003. Conference proceedings, abstract booklets, bibliographies of pertinent articles and books, and personal files were hand searched to identify additional articles. STUDY SELECTION Original investigations and reviews of the epidemiology of atherosclerosis and the association of conventional and novel risk factors with vascular risk were selected. On the basis of the search strategy, 373 relevant studies were identified. DATA EXTRACTION A diverse array of studies were examined, including randomized controlled trials, prospective cohort studies, systematic overviews, case-control, cross-sectional, and mechanistic studies. Data extraction was performed by one of the authors. DATA SYNTHESIS The available epidemiological and basic science evidence supports, to varying degrees, independent associations between these 4 candidate risk factors and atherosclerotic vascular disease. However, there is relatively little data regarding the additive yield of screening for these factors over that of validated global risk assessment strategies currently in use. Furthermore, controlled intervention studies targeting individuals with these factors for proven risk-reduction therapies, or specifically treating these factors with available therapies, are few. The explanatory power of the major, established cardiovascular risk factors has been systematically underestimated. CONCLUSIONS Although C-reactive protein, lipoprotein(a), fibrinogen, and homocysteine are associated with vascular disease risk, their optimal use in routine screening and risk stratification remains to be determined.

Can Allergic Disorders Decrease the Risk of Thromboembolic Events in Atherosclerosis? an Evidence-based Review
Atherosclerosis is a complex inflammatory disorder of the medium and large-sized arteries. Recent experiments demonstrated that atherosclerosis is a Th1 dominant autoimmune condition, whereas Th2 cells are rarely detected within the atherosclerotic lesions. Several studies have indicated that Th2 type cytokines could be effective in the reduction and stabilization of atherosclerotic plaque. The...
متن کاملThe Effects of l-arginine Supplementation on Athletic Performance and Risk Factors of Cardiovascular Disease: Review of Current Evidence
Introduction: Due to controversies in different studies about the effects of L-arginine supplementation on athletic performance and cardiovascular risk factors, this study aimed to review the current evidence on the effects of L-arginine supplementation on athletic performance and cardiovascular risk factors. Methods: This study was done to summarize findings from earlier evidence on the effect...
متن کاملارتباط پلیمورفیسم rs7887062 در Lnc-Ang362 با بیماریهای عروق کرونر آترواسکلروتیک در مبتلایان بدون فاکتورهای خطر
Background and purpose: A long non-coding transcript Lnc-Ang362 plays critical role on the expression of miR-221 and miR-222. Knockdown of Lnc-Ang362 reduces the expression of these miRNAs as well as vessel smooth muscle cell proliferation in response to vascular injury. This study, for the first time, considered the possible association between Lnc-Ang362 rs7887062A/G polymorphism with the ris...
متن کاملAssociation between Helicobacter pylori Infection and Atherosclerotic Coronary Artery Disease
Background and Objectives: Cardiovascular disease is the most common cause of death in developed countries. In addition to traditional risk factors for cardiovascular disease, nowadays, accumulating evidence indicates that a variety of infections contribute to pathogenesis of atherosclerosis, (We investigated whether Helicobacter pylori infection is related to Prevalence of coronar...
متن کاملConcept of Atherosclerosis Velocity: Is It a Better Measure of Cardiovascular Risk?
In most cases atherosclerosis is the underlying cause of vascular diseases, including heart disease and stroke. It is believed that endothelial injury is the earliest change in the artery wall and that this precedes the formation of lesions of atherosclerosis. Recent developments in the field of atherosclerosis have led to a renewed interest in the recognition of the parameter of time in the at...
